- It is a minimally invasive laparoscopic procedure where small holes are made in the ovarian surface using laser or electrocautery.
- This lowers androgen (male hormone) levels in the ovaries and can help restart natural ovulation.
When It’s Considered ?
- Women with PCOS-related infertility who do not respond to first-line medications like clomiphene, letrozole, or metformin.
- Those who want to avoid fertility drugs (which carry risks such as multiple pregnancies).
- Patients in whom other causes of infertility have been ruled out.
Step Process
Procedure
Step 1 :
- Done under laparoscopy (keyhole surgery) with general anesthesia.
Step 2 :
- A thin instrument with a camera is inserted into the abdomen.
Step 3 :

The surgeon uses either:
Electrocautery (heat probe)
Laser
To make 4–10 tiny punctures on the thickened ovarian surface.
Step 4 :
- This reduces androgen-producing tissue and helps restore hormonal balance.
Step 5 :
- Usually performed as a day-care procedure, meaning patients go home the same day.
Results & Benefits
- Improves ovulation:Around 50–80% of women resume regular ovulation within months.
- Pregnancy rates: Comparable to using fertility drugs, but without the high risk of multiple pregnancies.
- Reduces androgen symptoms: Some women also see improvements in acne and excess hair growth.
- Lower risk of ovarian hyperstimulation compared to fertility medications.
However, results may vary:
- Some women may not respond.
- There’s a small risk of scar tissue formation (adhesions) around the ovaries after surgery, which can affect fertility.
