Laparoscopic Excision
What It is ?
Laparoscopic excision surgery is a minimally invasive procedure where doctors use tiny incisions and a camera to see and remove endometriosis tissue. Unlike ablation (burning tissue), excision completely cuts out the lesions, which may give longer-lasting results.
When it is Considered ?
- When women experience severe pelvic pain, painful menstruation, or painful intercourse.
- When endometriosis is suspected to be a direct cause of infertility, e.g., blocking fallopian tubes or causing ovarian cysts.
- For women with ovarian endometriomas (chocolate cysts) that interfere with egg release.
- In moderate to severe disease confirmed by imaging or suspected clinically.
Procedure
Small Incisions
- A few tiny cuts are made in the abdomen.
Laparoscope Insertion:
- A thin camera is used to see inside the pelvic cavity.
Identification of Lesions:
- Endometriosis tissue, adhesions, or cysts are identified.
Excision:
- Lesions are carefully cut out and removed with specialized instruments.
Restoring Anatomy:
- Adhesions (scar tissue) are cut to free ovaries and tubes.
Ovarian Cysts:
- If present, cysts are drained and their capsule is removed.
Results
- Improves pain symptoms significantly, enhancing quality of life.
- Restores pelvic anatomy, giving a better chance for natural conception.
- Increases pregnancy rates, especially in mild and moderate cases.
- Limitation: If too much ovarian tissue is removed while excising cysts, ovarian reserve may decrease.
