Diagnosis
What It is ?
Diagnosis in this context means identifying whether endometriosis is present and whether it may be affecting fertility. Since symptoms can overlap with other pelvic conditions, proper diagnosis is crucial before deciding treatment.
Challenges in Dignosis
- Endometriosis can remain undiagnosed for years because symptoms like pelvic pain or irregular cycles are often dismissed as “normal period pain.”
- Some women may have minimal symptoms but severe infertility, making diagnosis more complex.
Methods of Diagnosis
Clinical Evaluation
- Doctors first take a detailed medical history, focusing on menstrual pain, pelvic pain, infertility duration, and family history.
- A physical (pelvic) exam may detect nodules, tenderness, or enlarged ovaries.
Imaging Techniques
- Ultrasound: Can detect ovarian endometriomas (“chocolate cysts”), but not always small lesions.
- MRI (Magnetic Resonance Imaging): Provides better visualization of deep infiltrating endometriosis
- These imaging tools help plan treatment but cannot confirm all cases.
Laparoscopy (Gold Standard)
- The most accurate method for diagnosis.
- A small camera is inserted into the abdomen to visually identify and sometimes biopsy lesions.
- Allows simultaneous treatment, lesions can be removed during the same procedure.
- Considered the best approach when infertility is suspected to be linked to endometriosis.
Histological Confirmation
- Biopsy samples taken during laparoscopy can be analyzed under a microscope to confirm the diagnosis.

Results of Diagnosis
- A confirmed diagnosis allows doctors to choose the right fertility treatment from medical therapy to surgical management or assisted reproduction.
- Early and accurate diagnosis improves the chances of restoring fertility and preventing further complications.
